fix(miniprogram): 精简菜单，移除推迟模块入口

- 个人中心：15→8 项菜单（移除积分商城/订单/用药/透析/知情同意/积分明细）
- 健康页：移除 BLE 设备同步入口
- 医生端：移除透析记录/透析处方快捷入口
